Background <p>As part of the “Gesundes Tal” (Healthy Valley) smart city project, the city of Wuppertal is developing a&#xa0;digital platform to improve individual health literacy and provide low-threshold access to health-promoting services. The aim was to record health-related lifestyle factors among citizens of Wuppertal in order to familiarize them with suitable services.</p> Methods <p>As part of a&#xa0;participatory, iterative process, a&#xa0;placement coach (named “all-around healthy”) was developed with questions from validated questionnaires on nutrition, exercise, sleep, wellbeing, and social environment. National and international guidelines, as well as differences to medical devices, were taken into account. Questions were selected according to age, some were changed into simple language, and they were combined into a&#xa0;composite questionnaire. Usability tests were conducted with various target groups.</p> Results <p>Questions on the topic of nutrition are based on recommendations from the DGE (German Nutrition Society), and questions are taken from the GEDA (“German Health Update”) and DEGS (“German Health Interview and Examination Survey for Adults”) studies conducted by the RKI (Robert Koch Institute). The topics of exercise and sleep are based on recommendations from the World Health Organization, and questions are taken from the KiGGS (“German Health Interview and Examination Survey for Children and Adolescents”) study. On the topic of social environment, adults are asked about emotional and practical support, while children and adolescents are asked about social support. The topic of mental health includes questions about mental well-being (SF-12) and stress, and for children and adolescents, about experiences of bullying (QBVQ). The results of the tests led to improved personalization, the integration of motivational elements, and a&#xa0;stronger local focus.</p> Conclusion <p>The developed placement coach allows for a&#xa0;resource-oriented assessment of health promotion-related needs and supports navigation across relevant platform-based services.</p>
