Certified exercise programs under § 20 SGB V are key elements of Germany’s preventive healthcare system. This study aimed to investigate the demographic, psychological, and health-related characteristics of course participants, and to identify subgroups with specific needs or burdens.
MethodsAn exploratory cross-sectional study was conducted with 87 participants of certified preventive exercise courses at a nonprofit organization in Frankfurt am Main. Standardized questionnaires assessed demographics, pain, resilience, stress, depression, self-efficacy, and health-related quality of life. A subgroup of 26 participants completed a follow-up survey after 3 months.
ResultsParticipants were predominantly female (85%) with a mean age of 57.6 years. Chronic pain was reported by 87%, with more than half experiencing it for over 5 years. Stress levels and resilience varied, with middle-aged individuals reporting notably lower resilience and self-efficacy. Intrinsic motivation and social interaction were key participation motives. Subgroups with high stress sensitivity and low resilience showed reduced quality of life. No significant changes were found in the longitudinal analysis.
ConclusionCertified courses currently attract mainly self-motivated and resilient individuals. To address public health equity, tailored approaches and course designs are required to better engage psychologically vulnerable groups.
