Monitoring der Implementierungsbereitschaft in Veränderungsprozessen

As a part of the project “Transsectoral Optimisation of Patient Safety” (TOP), an information technology (IT)-based intervention was implemented in hospitals with the aim of optimising the process of drug therapy for hospitalised patients with polypharmacy through intensified pharmaceutical care. Organisational readiness to implement is a crucial factor influencing their implementation. The aim of this article is to describe the development of readiness to implement in the participating hospitals over the observation period, to discuss the observed differences, and to present the implementation strategies derived from process monitoring.

Readiness for implementation was assessed using the “Checklist to Assess Readiness for Implementation” (CARI), which was adapted for the TOP project. Data were collected at five distinct time points, with the employees responsible for the project at the 12 participating hospitals. Data were analysed descriptively. The implementation strategies that were reviewed and expanded as part of the monitoring process were extracted from the project documents and categorised and described using the “Pragmatic Implementation Reporting Tool”.

The results on readiness to implement demonstrate a continuous improvement over time. Differences were observed between hospitals. Hospitals that commenced implementation of the intervention at an earlier stage demonstrated a greater readiness to implement than those with a shorter intervention period. The use of CARI for continuous monitoring also allowed for a review and tailoring of the implementation strategies.

It appears that CARI is an appropriate tool for monitoring the readiness to implement specific interventions over time. Continuous monitoring of readiness for implementation enables prompt identification of required support and the development of targeted strategies to improve the implementation process.
