<p>Gamma-hydroxybutyrate (GHB) is a central nervous system depressant with a high potential for dependence. Withdrawal can be severe and potentially life-threatening, often necessitating medical intervention. Evidence on outpatient detoxification remains limited. We present a case of a 41-year-old woman with severe GHB dependence to illustrate a structured, safe outpatient detoxification approach. This consisted of a gradual GHB tapering over 27&#xa0;days, supported by adjunctive pharmacotherapy and intensive clinical monitoring. She achieved abstinence at the end of the tapering regime and has maintained recovery for 7&#xa0;months to date. This case illustrates that outpatient detoxification can be feasible in carefully selected patients, whilst highlighting the need for standardised protocols and further research to optimise treatment strategies.</p>
