<p>This study estimated the proportion of nicotine dependence among current smokers in Korea and investigated its association with health behaviors. This cross-sectional study used data from the 2016, 2019, and 2022 Korea National Health and Nutrition Examination Surveys (<i>n</i> = 3,003 adult current smokers). The proportion of nicotine dependence among adult Korean current smokers, defined as Heavy Smoking Index ≥ 4, was 18.0%. In comparison with smokers showing low nicotine dependence, those with high nicotine dependence were more likely to engage in high-risk alcohol consumption (aOR = 1.51, 95% CI = 1.22–1.88) and less likely to undergo general health screening (aOR = 0.72, 95% CI = 0.56–0.91). Similarly, higher nicotine dependence scores were associated with more risky alcohol consumption (<i>p</i>-trend &lt; 0.001), general health screenings (<i>p</i>-trend = 0.002), and cancer screenings (<i>p</i>-trend = 0.017). Since smoking is a well-known major risk factor for diseases, a tailored health programs to encourage smoking cessation and improve health behaviors among smokers, especially those with high nicotine dependence, is essential.</p>
