<p>Depression is a major public health concern. Existing literature indicates a complex relationship between cigarette smoking and depressive symptoms. Previous studies have reported that relaxin (RLN) is associated with smoking and psychiatric diseases. This study aims to investigate the role of relaxin in the association between smoking and depressive symptoms. A case–control study was conducted with 167 Chinese adult males (69 smokers and 98 non-smokers). Relaxin levels in cerebrospinal fluid (CSF) were quantified, and the severity of depressive symptoms was assessed using Beck's Depression Inventory. Mediation analysis was performed to explore the relationship between smoking, CSF relaxin levels, and depressive symptoms. Smokers exhibited more severe BDI scores and higher CSF relaxin levels (<i>p</i> &lt; 0.001). There was a significant positive correlation between CSF relaxin levels and BDI scores in the overall population (<i>r</i> = 0.27,<i> p</i> &lt; 0.001). Additionally, a positive association was found between smoking and BDI scores (<i>β</i> = 0.375, bootstrap <i>95% CI</i> = 0.103—0.671, <i>p</i> = 0.009). CSF relaxin mediated the association between smoking and BDI scores in the mediation model (indirect effect value = 0.108, bootstrap <i>95% CI</i> = 0.021—0.202, <i>p</i> = 0.020), adjusted for age, BMI, marital status, and living conditions. This study highlights that CSF relaxin is associated with the relationship between smoking and depressive symptoms.</p>
