<p>Synthetic cannabinoids (SC) imitate the effects of natural cannabis and pose a particular concern for adolescents and young adults due to their low cost, legal ambiguity, and undetectability in standard drug tests. Yet, very little research has examined associations between using SC and natural cannabis (NC) in cannabis use disorder (CUD). The present study investigated associations of past 12-month SC and NC (SC-NC) polyuse with CUD in adolescent and young adult cannabis users using a comprehensive population sample. Using 2022 National Survey on Drug Use and Health (NSDUH) data, we examined a nationally representative sample aged 12 and older who reported past 12-month cannabis use (<i>N</i> = 14,271). We conducted descriptive analysis and logistic regression to evaluate the association of past 12-month SC-NC polyuse with the occurrence and severity of CUD. Sensitivity analyses were performed to reinforce the robustness of the main findings. Descriptive statistics indicated that SC-NC polyuse prevalence was higher among those with CUD compared to those without CUD. Multivariable logistic regression showed past 12-month SC-NC polyuse was significantly associated with CUD. Moreover, multinomial logistic regression revealed that SC-NC polyuse was a significant predictor of severe (vs. mild) CUD. Sensitivity analyses supported the main model findings. The findings of this study highlight significant associations between SC-NC polyuse and both the presence and severity of CUD. These results suggest the importance of developing targeted intervention to address SC use, which may help reduce the severity of CUD among cannabis users.</p>
