<p>Mental disorders are among the most common comorbidities in people with diabetes. Depending on the type and severity of the mental disorder, prior diagnosis and treatment, other comorbidities, and individual characteristics, numerous interactions between diabetes and mental health on the one hand and surgical procedures on the other must be taken into account. A&#xa0;number of these are addressed in this narrative review. In addition to highlighting increased somatic risks, especially in connection with severe mental illness, principles of medical communication, legal issues, and the role of psychotropic drugs are discussed. Furthermore, specific aspects that are relevant for the perioperative management of organic mental disorders as well as schizophrenic, affective, and addictive disorders are reviewed, taking into account current treatment guidelines. In often highly complex clinical situations, close interdisciplinary and interprofessional cooperation is in line with good clinical practice, which requires not only individual commitment but also structural anchoring.</p>
