<p>This case report describes myiasis, a wound infection caused by fly larvae, in a man with diabetes mellitus during a heatwave with outdoor temperatures of 32 °C. Extreme climatic conditions combined with prolonged exposure to flies facilitated the colonization of the wound by larvae, thus, underlining the importance of preventive measures and hygienic care during high temperatures. Diagnosis was confirmed through visual inspection and microbiological analyses to exclude secondary bacterial infections. Treatment included mechanical decontamination, antiseptic irrigation, and administration of antibiotics. The clinical course improved significantly after removal of the larvae and optimization of wound care. The case illustrates the pivotal role of preventive strategies in managing chronic wounds, particularly under extreme environmental conditions. This includes consistent wound coverage, protection against insects, and patient education about potential risks associated with heatwaves. Furthermore, it is increasingly important consider the impact of environmental factors, such as rising temperatures, on the pathogenesis of wound infections to ensure effective prevention and treatment.</p>
