Transition von Jugendlichen und jungen Erwachsenen mit Diabetes – immer noch eine Herausforderung

To ensure continuous diabetes care, long-term and close cooperation between pediatric and adult diabetes centers is necessary. This includes consideration of individual patient needs and wishes. A structured transition can help to improve metabolic control and continuous care. Despite many efforts regarding transition, there are still adolescents and young adults that drop out of diabetes care. This should be avoided especially in the view of possible acute complications and deterioration of metabolic control in this phase. Adolescents use pump treatment, continuous glucose monitoring, and automated insulin delivery systems more frequently than adults. Thus, there is a need of technical competence in adult treatment centers. The transition process should be prepared a timely manner with appropriate information and structured education regarding relevant themes. In the case of psychiatric comorbidity or psychosocial difficulties, comprehensive case management is useful.
