Update Typ-2-Diabetes

摘要

Important new insights into the diagnosis and treatment of type 2 diabetes are highlighted here. The focus is on pharmacological therapy according to current national and international guidelines. In patients with cardiovascular disease, initiation of pharmacological therapy with a sodium–glucose linked transporter 2 (SGLT2) inhibitor or glucagon-like peptide‑1 (GLP-1) receptor agonist, based on data from endpoint studies demonstrating superiority, is recommended. Particularly with incretin-based therapies, development continues to advance. The dual GLP-1/glucose-dependent insulinotropic peptide (GIP) receptor agonist tirzepatide is the first approved agent of a new generation of designer molecules that deliver their effects in a calculated manner through multiple receptors, thus, achieving favorable effects not only on glucose metabolism, but also on obesity and other comorbidities. An innovation in insulin therapy is the once-weekly administration of basal insulin, which can be an alternative to the previous daily injection for many patients with type 2 diabetes. With a focus on increasing therapy safety and achieving a more stable metabolic state, continuous glucose monitoring is also gaining importance in type 2 diabetes. The future will show whether automated insulin delivery (AID) systems will also become relevant in the treatment of type 2 diabetes.
