摘要

Background

Diabetes mellitus refers to heterogeneous diseases with differences in pathogenesis, disease progression and development of complications.

Research question

Do the (pre)diabetes subgroups differ in terms of insulin secretion and insulin clearance?

Materials and methods

Novel classification methods of (pre)diabetes for insulin secretion and clearance categorization are presented. Methods for determining insulin secretion and clearance are also described.

Results

The classification of prediabetes, type 1 and type 2 diabetes into subgroups based on simple clinical characteristics allows the identification of clusters with different insulin secretion. There are several methods to measure insulin secretion and clearance directly or indirectly. The severe autoimmune diabetes and severe insulin-deficient diabetes clusters showed the highest impairment of beta cell function and require insulin therapy more frequently than other diabetes subgroups.

Conclusion

New approaches that consider clinical, biochemical, and genetic characteristics can contribute to a more precise diagnosis and risk assessment as well as targeted prevention and therapy.
