The prevalence of metabolic dysfunction-associated steatotic liver disease (MASLD) is continuously increasing worldwide. Its progressive form, metabolic dysfunction-associated steatohepatitis (MASH), is a major risk factor for the development of hepatocellular carcinoma (HCC) in the fibrotic liver. Early detection of HCC is the most critical determinant of overall survival in patients with MASH, both with and without liver cirrhosis. Therefore, optimizing the currently insufficient screening algorithms is of paramount importance. Additive metabolic risk factors, such as obesity and type 2 diabetes mellitus along with genetic predisposition and alterations of the intestinal microbiome, may contribute to a better understanding of hepatocarcinogenesis in MASH. This review discusses the pathophysiological and epidemiological relationships between MASH and HCC, as well as available therapeutic options.
