Myocardial work and left ventricular function: implications for aerobic capacity and performance in professional soccer athletes

Myocardial Work (MW) has brought a new perspective on cardiovascular evaluation in athletes. In professional soccer athletes (PA), lower MW values have been observed compared to control individuals. However, the impact of the MW responses on cardiopulmonary and performance parameters in PA remains unclear. Thus, this study aims to evaluate LV function through the MW method in PA and explore the impact of the MW response patterns on aerobic capacity and treadmill performance in this population.
Methods75 PA and 23 recreational athletes (RA), between 18 and 35 years, underwent echocardiography and hemodynamic assessments. MW indices were obtained using specific software from pressure-deformation loops, constructed from noninvasive LV pressure curves. A subsample of the participants (20 PA and 10 RA) performed a treadmill cardiopulmonary exercise testing (CPET). The oxygen consumption (VO2) and speed (S) at the first ventilatory threshold (VT1) and the highest values (peak) reached in the CPET were analyzed.
ResultsThe global work index (GWI) was lower in PA (p = 0.005). Global constructive work (GCW), global wasted work (GWW) and global work efficiency (GWE) showed no difference between groups. The PA presented higher VO2 VT1 (p = 0.015), higher Speak and SVT1 (p = 0.000 and p = 0.000, respectively) compared to RA. No correlations were observed between MW, aerobic capacity and performance parameters.
ConclusionThe data indicate normal LV function in PA, based on preserved GWE. The GWI decrease is related to soccer-specific cardiac remodeling and seems not to impact the aerobic capacity or treadmill performance.
