Physical exercise as a modulator of ectonucleotidases activity in lymphocytes and cytokine levels in individuals on hemodialysis

Chronic kidney disease (CKD) is a disorder that can be caused by several health problems, manifested as a progressive reduction in the glomerular filtrate rate (GFR). In general, patients with CKD need to perform hemodialysis. Physical exercise is a strategy for preventing and treating chronic diseases because it has a positive impact on patients’ prognosis. We evaluated the effect of 12 weeks of resistance training protocol (RTE) on the activity of ectonucleotidases in lymphocytes of patients with CKD undergoing hemodialysis. Biochemical analysis of renal function, activity of the enzymes E-NTPDase, Ecto-5′-nucleotides, Adenosine deaminase (ADA) in lymphocytes were evaluated, before and after the RTE protocol. The levels of anti- and pro-inflammatory cytokines (TNF-α, IL-10, IL-6, IL-4, IL-2, INF-γ) were also evaluated. The RTE protocol increases GFR (p = 0.0154) and decreases creatinine (p = 0.0311) and K+ (p = < 0.0001) levels afterwards. Furthermore, we observed a decrease in ATP hydrolysis (p = 0.0214) and a decrease in the ADP hydrolysis (p = 0.0093), reduced AMP hydrolysis due to Ecto-5′-nucleotidase activity (p = 0.0482) and increased ADA activity (p = 0.0384) in lymphocytes. Interleukin 6 (IL-6) (p = 0.0357) and interleukin 4 (IL-4) (p = 0.0438) significantly decreased after the RTE protocol. Our findings highlight the beneficial effect of RTE in patients with CKD, reducing inflammation through purinergic signaling modulation and by reducing pro-inflammatory cytokine levels. In conclusion, due to their positive role RTE should be used as a non-pharmacological therapy in patients with CKD on hemodialysis.
