<p>Chronic kidney disease (CKD) is a disorder that can be caused by several health problems, manifested as a progressive reduction in the glomerular filtrate rate (GFR). In general, patients with CKD need to perform hemodialysis. Physical exercise is a strategy for preventing and treating chronic diseases because it has a positive impact on patients’ prognosis. We evaluated the effect of 12&#xa0;weeks of resistance training protocol (RTE) on the activity of ectonucleotidases in lymphocytes of patients with CKD undergoing hemodialysis. Biochemical analysis of renal function, activity of the enzymes E-NTPDase, Ecto-5′-nucleotides, Adenosine deaminase (ADA) in lymphocytes were evaluated, before and after the RTE protocol. The levels of anti- and pro-inflammatory cytokines (TNF-α, IL-10, IL-6, IL-4, IL-2, INF-γ) were also evaluated. The RTE protocol increases GFR (<i>p</i> = 0.0154) and decreases creatinine (<i>p</i> = 0.0311) and K<sup>+</sup> (<i>p</i> =  &lt; 0.0001) levels afterwards. Furthermore, we observed a decrease in ATP hydrolysis (<i>p</i> = 0.0214) and a decrease in the ADP hydrolysis (<i>p</i> = 0.0093), reduced AMP hydrolysis due to Ecto-5′-nucleotidase activity (<i>p</i> = 0.0482) and increased ADA activity (<i>p</i> = 0.0384) in lymphocytes. Interleukin 6 (IL-6) (<i>p</i> = 0.0357) and interleukin 4 (IL-4) (<i>p</i> = 0.0438) significantly decreased after the RTE protocol. Our findings highlight the beneficial effect of RTE in patients with CKD, reducing inflammation through purinergic signaling modulation and by reducing pro-inflammatory cytokine levels. In conclusion, due to their positive role RTE should be used as a non-pharmacological therapy in patients with CKD on hemodialysis.</p>
