Purpose <p>Postural assessment is essential for identifying and addressing musculoskeletal issues. Poor posture can impair biomechanics, movement efficiency, and overall well-being. Despite advancements in holistic assessment, current visual tools have not kept pace with scientific insights. This study addresses this gap by designing and validating the observational static posture assessment tool (OSPAT), which incorporates contemporary scientific understanding and best practices in musculoskeletal health.</p> Methods <p>This cross-sectional design includes recruiting qualitative and quantitative approaches to develop and test the psychometric properties of the observational static posture assessment tool (OSPAT). To establish the validity of the OSPAT, we employed Lawshe's content validity ratio (CVR) and content validity index (CVI) through a collaborative process involving consultation with 12 experts. Reliability was assessed by having 28 raters use OSPAT to evaluate 80 individuals for postural deviations (e.g., Upper Crossed Syndrome, Sway Back Posture) on a 6-point severity scale—Kappa coefficients (κ) quantified intra-rater and inter-rater reliabilities.</p> Results <p>Validity analysis showed a CVR of 0.70 and a CVI &gt;0.79. Reliability testing revealed substantial intra-rater agreement (<i>κ</i> = 0.68) and moderate inter-rater agreement (<i>κ</i> = 0.53).</p> Conclusion <p>OSPAT demonstrated strong validity and reliability for observational static postural assessment. Its holistic approach, addressing the interconnected nature of body systems, significantly improves traditional tools, facilitating better clinical evaluations and interventions.</p>
