<p>Theileriosis is a common tick borne hemoprotozoan disease of cattle in India and the World. Currently, Buparvaquone is the most commonly used drug for the treatment and prevention of Theileriosis in Bovines. However, recent reports of Buparvaquone resistance have been made in several endemic regions. This study aimed to evaluate clinical, hematological and biochemical alterations in Theileriosis infected cattle and compared the efficacy of <i>Peganum harmala</i> and <i>P. harmala</i> along with Oxytetracycline with Buparvaquone. The cattle were screened and 18 Theileriosis positive cases were selected based on thin blood smear examination which were randomly divided into three groups (<i>n</i> = 6). Group I was treated with inj. Buparvaquone @ 2.5&#xa0;mg/kg BW IM, group II was treated with <i>P. harmala</i> @ 7&#xa0;mg/kg BW IM and group III was treated with <i>P. harmala</i> @ 7&#xa0;mg/kg BW IM along with inj. Oxytetracycline @ 20&#xa0;mg/kg BW IV. All the infected cattle were treated with specific and supportive therapy along with vitamin B - complex for 3 days. Theileriosis infected cattle showed tick infestation, swollen lymph nodes, fever, pale mucous membrane, anorexia, nasal discharge, dullness, depression and weakness. All three groups showed significant (<i>P</i> &lt; 0.05) improvement after treatments in clinical parameters (heart rate, rectal temperature and respiratory rate), hemato-biochemical parameters (total erythrocyte count, packed cell volume, absolute neutrophil, absolute monocyte, total protein, globulin, ALT, AST, BUN and creatinine) and reduction in parasitemia indicating promising effect against Theileria parasite. Group I, II and III showed 100%, 66.66% and 83.33% therapeutic efficacy respectively against theileriosis. Hence, <i>Peganum harmala</i> @ 7&#xa0;mg/kg BW IM can be used for treatment of Theileriosis in cattle.</p>
