Primary Multiple Hypertensive Intracerebral Hemorrhages

Non-traumatic intracerebral hemorrhages of multiple locations most often occur in the presence of coagulopathies, uncontrolled use of anticoagulants, vasculitis, venous sinus thrombosis, amyloid angiopathy, and other conditions. If secondary risk factors cannot be identified, these hemorrhages are regarded as primary multiple hemorrhages due to hypertension. Their pathogenesis is poorly understood, with most researchers suggesting sequential vascular ruptures, where the first hemorrhage increases intracranial and arterial pressure, potentially triggering the development of another hemorrhage. Few cases of primary multiple intracerebral hemorrhages have been described, so the optimal management strategy for such patients has not been determined. This report presents two clinical cases of patients with primary multiple hypertensive intracerebral hemorrhages, along with a brief review of current publications on this topic.
