Tongue and Epiglottis Surgery in the Treatment of Obstructive Sleep Apnea Syndrome

This review of the literature describes treatment options for obstructive sleep apnea syndrome (OSAS) caused by upper airway soft tissue collapse. Emphasis is placed on the importance of thorough examination of the patient, including sleep video endoscopy (SVE) to determine the exact level of obstruction for selection of the optimal surgical strategy. The paper discusses various techniques for surgery on the root of the tongue and epiglottis, including submucosal minimally invasive lingual resection (SMILE), transoral robotic tongue surgery (TORS), low-temperature tongue root coblation (LTBR), and others. Indications for tongue root and epiglottis surgery are analyzed, as are the advantages and disadvantages of surgical methods as compared with traditional continuous positive airway pressure (CPAP) therapy. The results of this literature review lead to the conclusion that surgical interventions on the root of the tongue and epiglottis can be used for certain indications in adult patients with snoring and OSAS.
