<p>This article presents the epidemiology of stroke and dementia based on data from the Moscow region, the Russian Federation, Western Europe, and the USA. A brief review of the literature on primary progressive aphasia (PPA) is provided and the diagnostic criteria and main variants of the disease are described. Two clinical observations are presented: a patient with an agrammatic form of PPA and a patient with post-stroke aphasia. Results from studies of routine MRI, functional MRI with a speech paradigm, and diffusion tensor imaging with tractography are presented and the main differences in the clinical picture and MRI investigation results in cases with a similar clinical picture of motor aphasia caused by a neurodegenerative process and developing as a result of stroke are analyzed. Additional tests for early detection of the agrammatic form of PPA are given.</p>
