A Six-Factor Model of the PANSS

Objectives. To analyze the factor structure of PANSS in a cohort of inpatients with schizophrenia and to compare the resulting factors with data from neuropsychological assessment of their cognitive functions. Materials and methods. The study included 110 patients (50 women and 60 men) with established diagnoses of schizophrenia. Psychopathological symptoms were assessed using the PANSS scale in the Russian-adapted version – SCI-PANSS. Indicators of cognitive functioning were assessed using the Brief Assessment of Cognition in Schizophrenia (BACS) scale. Results. Principal component analysis was used to construct a six-factor model of the PANSS, which included a negative factor, a positive factor, a depressive factor, an excitement factor, a disorganization factor, and a cognitive factor. The cognitive factor was the only factor associated with some of the subtests and with the composite BACS score. Negative correlations were found with digit sequencing (r = –0.184, p = 0.045), the token motor task (r = –0.250, p = 0.019), the verbal fluency test (r = –0.186, p = S), the symbol coding test (r = –0.291, p = 0.002), and the composite BACS score (r = –0.228, p = 0.017). Conclusion. The value of the PANSS as an assessment tool for the severity of cognitive impairment in patients with schizophrenia can be increased by using a six-factor model with differentiation of the disorganization and cognitive factors.
