Neurological Complications Resulting from Nitrous Oxide Intoxication

Nitrous oxide (NO) intoxication is common among young people who use this substance for recreational purposes. One serious consequence is the development of myelopolyneuropathy, where polyneuropathy symptoms are more common and onset is acute or subacute. The development of neurological complications due to NO intoxication is mediated by vitamin B12 deficiency, though laboratory confirmation of this condition involves a number of issues. A critical aspect of the treatment of neurological disorders caused by NO intoxication is administration of therapeutic doses of vitamin B12 over prolonged time periods. A clinical case is presented demonstrating the difficulties of confirming vitamin B12 deficiency in these patients and the advisability of prescribing oral cyanocobalamin.
