Background <p>Glaucoma is one of the most important causes of irreversible blindness around the world, and it has significant consequences for the vision-related quality of life (QoL) for the patients. There are few studies examining glaucoma patients’ subjective experiences—particularly treatment satisfaction and vision-related QoL—in Pakistan and comparable South Asian, resource-limited settings. This study aims to assess the perceived QoL and treatment satisfaction among the glaucoma patients. More emphasis has been given to the impact of glaucoma treatments and their side effects.</p> Materials and methods <p>A cross-sectional study was conducted among 277 glaucoma patients. Participants were recruited from ophthalmology outpatient clinics and completed a structured questionnaire, including the NEI-VFQ-25 (National Eye Institute Visual Function Questionnaire-25), to assess vision-related QoL, questions about treatment satisfaction, side effects, and the affordability of medications. Descriptive statistics were used to summarize demographic data, medical history, and treatment outcomes. Chi-square tests and logistic regression models analyzed associations between treatment satisfaction, side effects, and QoL outcomes.</p> Results <p>Most of the patients were very satisfied with their treatment for glaucoma, as in fact, it lowers IOP. Two significant side effects-ocular redness in 29.24% and blurred vision when instigating the drops in 24.19%-had a negative impact on treatment satisfaction and adherence. There was an essential impairment in QoL concerning difficulties with near, distance and side visions. There were socioeconomic issues as well, since 13.36% of the patients reported a difficulty affording medications.</p> Conclusion <p>Glaucoma significantly affects the QoL in patients enrolled in this study. Difficulty with vision and treatment adverse effects remain significant sources of dissatisfaction and non-adherence. The results stress the need for a more holistic, patient-oriented approach in glaucoma management, taking into account the clinical outcomes together with the patient-reported experiences of side effects and economic burdens of the treatment. Addressing all these factors will be essential to improve QoL and treatment adherence in resource-limited settings.</p>
