Purpose <p>To compare gas and silicone oil (SO) tamponades on inferior quadrant recurrence in patients undergoing pars plana vitrectomy (PPV) for rhegmatogenous retinal detachment (RRD).</p> Methods <p>This retrospective study included patients who underwent PPV with intraocular tamponade for RRD between May 2019 and February 2023. Patients with proliferative vitreoretinopathy (PVR) grade C1 or lower and at least three months of follow-up were included. Patients were divided into two groups based on the use of gas or SO tamponade. Comparisons were made regarding age, gender, lens status, intraocular pressure (IOP) changes, recurrence rates, tear localization, number of surgeries, and final anatomical and visual success.</p> Results <p>A total of 121 eyes of 121 patients were included, 59 in the gas group and 62 in the SO group. Inferior quadrant recurrence occurred in 7 (11.2%) patients in the gas group and 6 (10.1%) in the SO group (<i>p</i> = 0.842). The mean time to recurrence was 24 ± 11.5&#xa0;days for the gas group and 31.3 ± 9.8&#xa0;days for the SO group (<i>p</i> = 0.345). Mean BCVA improved significantly in both groups (<i>p</i> &lt; 0.01). The mean number of surgeries was 1.16 ± 0.48 in the gas group and 2.18 ± 0.57 in the SO group (<i>p</i> &lt; 0.001). IOP elevation requiring treatment occurred in 15.3% of the gas group and 34% of the SO group (<i>p</i> = 0.024).</p> Conclusion <p>There was no significant difference in aspect of inferior quadrant recurrence between gas and SO in patients without advanced PVR. Silicone oil lead to additional surgeries and complications.</p>
