<p><i>Taxus wallichiana</i> Zucc. or Himalayan Yew; is a perennial tree belonging to the family Taxaceae. It is mainly found in the Himalayan region and has long been used in traditional medicinal system. The bark and needles of this plant are commonly used by ethnic people to make tea, along with other traditional uses to treat many diseases. <i>Taxus wallichiana</i> is a vital source of pharmacologically active compounds, notably paclitaxel. However, significant gaps remain in clinical validation, detailed research on its biosynthetic pathways, and the establishment of standardized quality control measures. Given its rising demand and the decline of wild populations, there is an urgent need to prioritize its conservation through sustainable cultivation, regulated harvesting. All available information on <i>Taxus wallichiana</i> was collected via electronic searches using PubMed, ResearchGate, ScienceDirect, Semantic Scholar, Scopus, Google Scholar, and Web of Science. More than 170 compounds have been identified from this species, including taxanes, diterpenoids, lignans, flavonoids, and phenolics. Nonetheless, quality control is challenged by risks of adulteration and variations caused by environmental factors. To the best of our knowledge, this is the most comprehensive summary to date of the phytochemicals, traditional uses, pharmacology, structure–activity relationships, and quality control of <i>Taxus wallichiana</i>. The documented pharmacological properties not only underscore its therapeutic potential but also provide a strong scientific basis for developing plant-based drugs and advancing future ethnopharmacological research. Future research should place greater emphasis on elucidating concrete pharmacological mechanisms.</p> Graphical Abstract <p></p>
