<p>In Global South, sanitation and water management remain pressing challenges, marked by structural inequalities, institutional fragility, and growing climate risks. Climate change exacerbates these conditions, increasing the need for effective public policies and analytical tools capable of holistically assessing municipal management efficiency, integrating both service provision and water risk mitigation. This study aims to propose a Water Efficiency, Risk and Resilience Index (WE2RI) model to assess municipal efficiency in promoting sanitation and managing water risks. It uses exploratory factor analysis (EFA) and data envelopment analysis (DEA), specifically the results-oriented DEA-CCR model, to measure performance and propose a synthetic indicator. The former technique reduces data dimensionality and identifies key dimensions, while the latter assesses efficiency and performs benchmarking analysis. This study applies the model to Brazil, focusing on the Water Resources Management Unit (UGRHI 5). Its application demonstrated its potential to identify benchmarks, classify municipalities by performance, and offer subsidies for more efficient and resilient public policies. In addition, WE2RI contributes to municipal public policies by making it possible to: (i) calculate overall efficiency scores and scores by dimension; (ii) identify benchmarks and improvement trajectories; (iii) define performance targets; and (iv) analyze municipal performance in a disaggregated or aggregated manner. Although applied to a Brazilian case, the model contributes to the international debate by offering a methodological design that can be replicated and compared in different contexts, especially in countries of the Global South that face similar challenges of inequality, climate vulnerability, and institutional fragility in the sanitation sector.</p>
