A novel search approach for secure and flexible sharing of hierarchical EHR based on blockchain

Electronic Health Records (EHRs) contain sensitive health data of users, which are shared across different institutions to improve healthcare quality. As a result, privacy leakage risks have intensified. While the existing schemes using attribute-based encryption (ABE) can effectively protect the sharing information, most of them still fail to access data with high efficiency and meet fine-grained protection requirements for owners’ sensitive privacy preferences regarding their EHRs. To address these issues, we propose a novel Search-Hierarchical Access-Search (SHAS) approach organically combining of a Blockchain-based Searchable Hierarchical Attribute-Based Encryption (BS-HABE) scheme and an Inverted Index-based Dual Search (IIDS) algorithm. The proposed BS-HABE scheme addresses the sensitivity of data privacy in fine-grained hierarchical sharing. BS-HABE enables secure, flexible, and efficient sharing of EHRs with varying sensitivity levels by incorporating a privilege-based access structure and supporting scalable hierarchical data management. The designed IIDS algorithm enhances the efficiency of encrypted data retrieval by integrating a dual-search approach, allowing comprehensive and precise searches across hierarchical EHRs. Extensive experiments further demonstrate that the proposed approach significantly reduces storage costs and optimizes search efficiency, outperforming existing solutions in these critical aspects.
