<p>We report a rare case of idiopathic hypereosinophilic syndrome presenting with isolated cardiac involvement in the form of sub-mitral intracardiac thrombus leading to severe functional mitral stenosis and severe eccentric mitral regurgitation. This case highlights the role of multi-modality imaging in the evaluation of intra-cardiac thrombi at unusual locations, especially in hypereosinophilic syndromes and the need of prompt and aggressive management in patients with severe valvular involvement for better clinical outcomes.</p>
