Second primary cancers among females with a first primary cervical, uterine and ovarian cancer: a population-based study in North Portugal

To estimate the incidence rates of second primary cancers (SCPs) [diagnosed > 2 months after a first primary cancer (FPC)], and cumulative incidence and mortality of metachronous SPCs in patients with a cervical, uterine, or ovarian FPC, and to compare the incidence of SPCs with that expected in the general female population.
MethodsA cohort of patients with cervical, uterine, or ovarian FPC from the North Region Cancer Registry of Portugal, diagnosed in 2000–2010 (n = 5846) was followed until 31/12/2015 for synchronous and metachronous SPCs. Cumulative incidence of metachronous SPCs considering death as a competing event, and incidence rates and standardized incidence ratios (SIR) of SPCs were estimated.
ResultsDiagnosis of an SPC occurred in 541 (9.3%) patients at a median age of 60 years. SPCs were more frequent among patients with uterine cancers. Colon, breast, and corpus uteri were the most common sites for an SPC among patients with a gynecological FPC. A higher incidence of all and individual SPCs compared to the general female population was found (SIR = 1.96, 95% CI 1.79–2.12) with the most relevant sites being cancers of the colon (SIR = 2.32, 95% CI 1.82–2.89), lung (SIR = 2.70, 95% CI 1.88, 3.66), cervix (SIR = 4.87, 95% CI 3.43, 6.56), corpus uteri (SIR = 5.37, 95% CI 4.16, 6.73) and ovary (SIR = 6.06, 95% CI 4.12–8.37). The 10-year cumulative incidence of a metachronous SPC and death for patients with a gynecological FPC was 7.4% and 39.2%, respectively.
ConclusionIn Portugal, patients with a gynecological FPC have a higher incidence of SPCs compared to the general female population, particularly elderly females. Increased surveillance and counselling among these patients are needed.
