Background <p>Chronic obstructive pulmonary disease (COPD) is a&#xa0;common, clinically heterogeneous condition requiring lifelong medical treatment. This field has substantially evolved in recent years. Alongside the use of established bronchodilator strategies, new treatment options with biologicals are becoming increasingly more important.</p> Research question <p>Presentation of current concepts in pharmacological COPD treatment, taking into account established therapeutic principles and new developments based on the current German S2k guidelines on&#xa0;specialist diagnostics and treatment of chronic obstructive pulmonary disease (2026).</p> Material and methods <p>A&#xa0;narrative review of current evidence-based treatment recommendations, with particular reference to the German S2k guidelines on specialist diagnostics and treatment of COPD (2026) as well as selected clinical trials and review articles.</p> Results <p>The pharmacotherapy of COPD is primarily focused on symptom control, improving the quality of life and reducing exacerbations and disease progression. Long-acting anticholinergics (LAMA) and long-acting beta 2 agonists (LABA) are the preferred choice for dual bronchodilation. Treatment escalation with inhaled corticosteroids (ICS) is beneficial in patients with exacerbations and an eosinophil count ≥ 100/µl. In cases of persistent exacerbations despite optimal inhaled treatment, additional options such as phosphodiesterase-4&#xa0;inhibitors or biologicals as anti-inflammatory therapies can be considered. Recent developments are increasingly opening up new treatment options with biologicals for patients with COPD.</p> Discussion <p>The treatment of COPD is shifting from a&#xa0;stepwise to an individualized approach. Alongside new pharmacological options, the correct selection and use of extended therapeutic options are a&#xa0;decisive factor for treatment success.</p>

Background

Alongside the use of established bronchodilator strategies, new treatment options with biologicals are becoming increasingly more important.

Research question

Presentation of current concepts in pharmacological COPD treatment, taking into account established therapeutic principles and new developments based on the current German S2k guidelines on specialist diagnostics and treatment of chronic obstructive pulmonary disease (2026).

Material and methods

A narrative review of current evidence-based treatment recommendations, with particular reference to the German S2k guidelines on specialist diagnostics and treatment of COPD (2026) as well as selected clinical trials and review articles.
