Low-dose computed tomography (LDCT) lung cancer screening is a newly implemented national early detection initiative in Germany, targeting individuals at high risk. Evidence from the major trials demonstrates a substantial shift towards earlier, more treatable tumor stages and a significant reduction in lung cancer mortality. The program is legally based on the German Lung Cancer Early Screening Regulation (LuKrFrühErkV). It follows a multistep approach with initial counselling/eligibility assessment followed by LDCT imaging in certified radiology centers. Findings are categorized using the standardized Lung-RADS system. Suspicious or positive findings can trigger earlier follow-up or a comprehensive evaluation at a lung cancer center. In addition to the oncological benefits, the program emphasizes the management of incidental findings and counselling on smoking cessation. Pulmonologists play a central role in risk assessment, patient counselling, interdisciplinary case management and coordination of subsequent treatment.
