Lung transplantation is an established treatment option for selected patients with advanced lung diseases. The long-term success crucially depends on a structured follow-up care, which addresses both transplantation-specific complications and comorbidities under immunosuppression and is carried out in close coordination with the transplantation center. Key components of the aftercare include consistent monitoring of graft function, prompt diagnostic work-up in the event of suspected graft dysfunction and prevention and management of infectious and noninfectious complications. This article summarizes practical aspects of follow-up after lung transplantation and classifies them in the context of the 2025 guidelines. The aim is to support the early recognition of complications and ensure safe care, including settings outside the transplantation center.
