Key components of the aftercare include consistent monitoring of graft function, prompt diagnostic work-up in the event of suspected graft dysfunction and prevention and management of infectious and noninfectious complications. This article summarizes practical aspects of follow-up after lung transplantation and classifies them in the context of the 2025 guidelines. The aim is to support the early recognition of complications and ensure safe care, including settings outside the transplantation center.</p>
