Background <p>Systemic lupus erythematosus (SLE) can affect practically all organ systems. This also applies to the thoracic organs, which can be affected in very different ways.</p> Objective <p>To describe the various pleural and pulmonary manifestations of SLE with a&#xa0;focus on the holistic management of patients with the disease.</p> Material and methods <p>A nonsystematic interdisciplinary literature review on the diagnosis and management of pleuropulmonary manifestations was carried out.</p> Results <p>In contrast to polyserositis with pleuritic pain and pleural effusions, which is relatively frequent at 16%, pulmonary and pulmonary vascular organ manifestations are rare, all in the low single figure percentage range. These include shrinking lung syndrome, lupus pneumonitis and diffuse alveolar hemorrhage, interstitial lung disease (SLE-ILD) and various forms of pulmonary hypertension. In patients with SLE various extrapulmonary manifestations often have to be treated in parallel. The treatment is mostly interdisciplinary and includes a&#xa0;range of other measures in addition to immunosuppression. In some respects the treatment is the same for all SLE patients (e.g., hydroxychloroquine) but in other aspects it is tailored to the treatment of specific manifestations (e.g., anticoagulation with vitamin&#xa0;K antagonists in antiphospholipid syndrome).</p> Conclusion <p>With interdisciplinary cooperation and knowledge of the complex pathophysiological relationships, the life-threatening pulmonary manifestations of SLE can nowadays mostly be successfully controlled.</p>

Background
