Venovenöse extrakorporale Membranoxygenierung (VV‑ECMO) und schweres akutes Lungenversagen (ARDS)
摘要
Acute respiratory distress syndrome (ARDS) is a disease with a high mortality rate. Venovenous extracorporeal membrane oxygenation (VV-ECMO) as a lung support procedure can improve patient survival in severe ARDS. Current European and US guidelines recommend the use of VV-ECMO in severe ARDS. Stringent patient selection and consideration of contraindications are essential. Specific and close monitoring is necessary during VV-ECMO treatment due to the numerous and frequent complications.
Even after several months of support with VV-ECMO, promising survival rates and a satisfactory long-term functional outcome are possible. Due to the complexity of the patients and the procedure as well as the severity and the multitude of possible complications, treatment in designated centers is meaningful and necessary. Extensive experience in the treatment of ARDS and management with ECMO in combination with essential structural requirements is indispensable to qualify as a center.
