Cardiopulmonary exercise testing (spiroergometry) is an established instrument to evaluate the complex performance (patho)physiological components of metabolism, circulation and ventilation during physical exercise. Ventilation is of paramount importance for the assessment of the workout, the diagnosis, establishing disease severity and monitoring therapy in pulmonary diseases with obstructive or restrictive dysfunction and also in diseases of the pulmonary and systemic circulations. For modern patient care profound knowledge of the quantitative and qualitative aspects of ventilation during exercise are mandatory. This article provides relevant insights into ventilation during exercise and can be of practical help when interpreting the results of cardiopulmonary exercise testing.
