<p>We retrospectively analyzed more than 17&#xa0;million routinely collected laboratory data sets from 2017 to 2022 to determine the annual mean values for total, LDL and HDL cholesterol and triglycerides in women and men. An interrupted time series analysis was used to investigate the effects of lockdowns on lipid levels.</p><p>Most lipid values were within the recommended ranges, with the exception of triglycerides in men (176.8 mg/dl in 2022). During the observation period, average LDL cholesterol levels fell by a&#xa0;significant and clinically relevant magnitude (from 131.2 mg/dl to 120.4 mg/dl in women and from 126.9 to 114.8 mg/dl in men). For the periods of lockdowns during the COVID-19 pandemic we observed a&#xa0;slight but statistically significant increase in triglycerides of 3.80 mg/dl (95% confidence interval 1.18–6.43 mg/dl) and also a&#xa0;monthly trend change in LDL cholesterol levels of 0.18 mg/dl (95% confidence interval 0.05–0.31 mg/dl).</p><p>The COVID-19 lockdowns had only a&#xa0;minor unfavorable impact on lipid profiles in Germany. Positive developments in LDL cholesterol may be explained by updated prevention measures, including new therapeutic guidelines and the adaptation of the screening guideline.</p>
