Epilepsiechirurgische Therapie hypothalamischer Hamartome

Hypothalamic hamartomas (HH) are rare congenital lesions that often lead to pharmacoresistant epilepsy and progressive encephalopathy. The surgical treatment has evolved from open resection approaches to minimally invasive disconnection techniques. Recent meta-analyses of over 1500 patients show that laser interstitial thermotherapy (LITT) and stereotactic radiofrequency thermocoagulation (RFTC) achieve comparable seizure-free rates between 69–87%. Stereotactic radiosurgery (SRS) is lower at 39–44% with a lower risk profile. Volumetric studies suggest that success depends more on the anatomical precision of the disconnection than on the size of the region treated or the technology used. Globally, RFTC provides the greatest potential for worldwide implementation of HH treatment due to broader technological availability.
