<p>Seizures are the most frequent neurological emergency in neonates. Neonatal seizures are mostly acutely provoked but in ca. 15% they can be the primary manifestation of an epilepsy with neonatal onset. With regards to antiseizure medication, specific physiological aspects of the neonatal central nervous system have to be taken into account, as well as pharmacokinetic and ethical issues. This review is based on the guidelines and recommendations for the treatment of seizures in neonates published by the International League Against Epilepsy (ILAE) in 2023, supplemented by current studies.</p>
