Background <p>In June 2021 the European Society of Human Reproduction and Embryology (ESHRE) published guidelines on the topic of medically assisted reproduction in patients with viral diseases. The long version consisting of 174 pages reports on 7 different types of virus. This has never previously been published in guidelines. In the clearly produced tables, even in the short version, it is quickly recognizable which aspects require special consideration; for example, ejaculate preparation and testing for the virus, the need for vaccination or the necessity of a&#xa0;cesarean section or the question of breastfeeding. The methodological effort was immense and the preparation time was nearly 4 years. Improved treatment approaches for numerous viral infections as well as the access to reproductive medicine treatment strategies for people wishing to have children have become easier; however, there is still a relative uncertainty among treating physicians regarding exactly what should be considered for each type of virus. Therefore, the present guidelines provide a&#xa0;valuable aid for the daily practice.</p> Objective <p>The development of the guidelines is presented, emphasizing that previously only human immunodeficiency virus (HIV) infection was considered in guidelines. Now, however, six different types of virus are analyzed.</p> Material and methods <p>As a co-author of the guidelines, the author of this article can report on the process of its preparation.</p> Results and conclusion <p>Compared to guidelines that are up to 20&#xa0;years older, it can be observed that the approach to a HIV infection in particular has become significantly more objectified. Initial fears, such as with respect to liability issues, are no longer paramount; however, well-trained personnel, adequate equipment and comprehensive patient clarification are relevant for all viral diseases addressed in the guidelines.</p>
