<p>Adenomyosis is a&#xa0;severe disease picture, which is accompanied by endometriosis. In many cases chronic pain and substantial bleeding disorders lead to a&#xa0;hysterectomy. This is not an option in cases of an unfulfilled desire to have children. Pathophysiologically, adenomyosis is a&#xa0;highly complex cause of infertility. In the diagnostics and treatment of infertility this complexity must be adequately taken into account both with respect to a&#xa0;possible spontaneous conception as well as during treatment with assisted reproductive technology (ART) procedures. Can a&#xa0;fertility-preserving surgical procedure lead to success and also to a&#xa0;reduction of the complaints and a&#xa0;subsequent conception? According to the current state of knowledge pharmaceutical treatment approaches or ART techniques are given priority; however, surgical strategies should also be considered to enable an individualized treatment.</p>
