In people with gender incongruence there is a discrepancy between the gender assigned at birth and the perceived gender identity. With the onset of puberty many transgender people experience a significant reduction in the quality of life. Studies show that gender incongruence is often associated with significant negative effects on mental health. Gender-affirming hormone therapy (GAHT) can alleviate psychological symptoms and improve the quality of life. A GAHT should be planned according to the individual’s needs and detailed information about the possibilities and limitations of GAHT is recommended. Due to the far-reaching consequences of sex-affirming therapy on fertility, information and advice on the possibilities of fertility protection in the event of a later desire to have children should be provided before starting GAHT.
