Background

The synchronization between embryonic development and the receptive state of the endometrium significantly influences the success of assisted reproductive techniques (ART). Endometrial gene expression analysis was introduced to determine the optimal time for embryo transfer using molecular biology.

Aim

Does an endometrial expression analysis improve the reproductive outcome and is the application in clinical practice justified?

Material and methods

Overview of relevant publications on the topic of endometrial receptivity analysis.

Results

The studies published so far on the reproductive outcomes of personalized vs. standardized embryo transfer mostly have a retrospective study design. Among the published prospective studies, the study by Doyle et al. proved to be important: it showed that the use of personalized embryo transfer in a cohort of patients with a good prognosis does not improve the reproductive outcome. There is a lack of high-quality data to answer the question of whether this also applies to a cohort with recurrent implantation failure.

Conclusion

Based on the research results to date, the routine use of endometrial gene expression analysis is not recommended. It is possible that a prospective study announced for 2026 will provide new evidence that will enable its use in patients with repeated implantation failure.
