<p><i>Helcogramma flammata</i> sp. nov. (Blenniiformes: Tripterygiidae), described on the basis of 43 specimens from the Izu and Ogasawara islands, and Pacific coast of the Japanese mainland, is characterized by the following combination of characters: 36–40 (modally 38) scale rows in longitudinal series; mandibular pore formula usually 4 + 1 + 4; orbital tentacle extremely short, unbranched; blue stripe extending from lower margin of orbit through opercle; body of pale males and females yellow, with distinct brownish vertical bands; and body of nuptial males bright orange, with black lips, cheek and pectoral-fin base. The new species is most similar to the Fourspot Triplefin <i>Helcogramma fuscipectoris</i> (Fowler 1946), with which it has previously been mis-identified. However, the new species is clearly distinguishable in having 10–12 (modally 11) third dorsal-fin rays [vs. 9–11 (10) in <i>H. fuscipectoris</i>]; 22–25 (24) pored lateral-line scales [vs. 19–23 (22)]; and 5–8 (usually 6) suborbital pores [vs. 5–7 (usually 5)]. Although the nuptial males of both species share a black mask and blue stripe on the head, such black pigmentation on the head is restricted to the lips in <i>H. flammata</i> sp. nov. (vs. black area extending to the snout and anteroventral margin of the eye in <i>H. fuscipectoris</i>). In addition, the blue stripe in nuptial males of the new species extends from the lower margin of eye to the opercle, whereas it extends horizontally from the posterior tip of the maxilla through the preopercular margin in <i>H. fuscipectoris</i>. Moreover, the genetic analysis based on the mtDNA COI gene clearly separated <i>H. flammata</i> sp. nov. and <i>H. fuscipectoris</i>, with a genetic distance of 14.7–16.0% between them.</p>
