A randomised comparative study of 1064 nm Nd:YAG laser and Urea occlusion therapy with topical miconazole for the treatment of onychomycosis

Onychomycosis is a chronic fungal infectious disease and its treatment remains quite challenging at present. We aimed to evaluate the clinical efficacy of 1064 nm Nd:YAG laser in the treatment of onychomycosis. In this study, patients were randomly divided into three groups: laser (L), urea occlusion (UO), laser combined with urea occlusion (L + UO). Group L was treated with 1064 nm Nd:YAG laser. Group UO was treated with urea encapsulation. Group L + UO was treated with urea encapsulation followed by laser. All patients received Miconazole nitrate cream twice a day. Follow-up was conducted at 12 and 24 weeks. The Scoring Clinical Index for Onychomycosis (SCIO) and mycological examination were conducted. The clinical efficacy was evaluated based on the SCIO scores and the “temporary clear nail increase rate”. A total of 59 patients with 110 affected nails were initially enrolled. Two patients (5 nails) were lost to follow-up, leaving 57 patients and 105 nails that had completed follow-up. At 24 weeks, in Group L, Group UO, and Group L + UO, the SCIO response rates were 42.86%, 66.67%, and 61.11%, respectively, the “temporary clear nail increase rates” were 19.05%, 16.67%, and 47.22%, respectively, and the mycological cure rates were 26.52%, 53.33%, and 56.52%, respectively. The “temporary clear nail increase rate” between Group L and Group L + UO, and the average SCIO before and after treatment in three groups was statistically significant. In conclusion, the combination of 1064 nm Nd:YAG laser and UO is an effective treatment for onychomycosis, which is of great significance for improving the therapeutic efficacy of onychomycosis.
