In vitro activity of β-lactams against contemporary Enterococcus faecalis clinical isolates and performances of disc diffusion testing for prediction of β-lactam susceptibility

This study evaluates the in vitro activity of five clinically-relevant β-lactam antibiotics against 158 Enterococcus faecalis clinical isolates from France and assesses whether ampicillin (AMP) disc diffusion can predict susceptibility. No β-lactamase-producing strains were detected and 100% of them remained susceptible to amoxicillin. By contrast, resistance was detected for piperacillin, imipenem, meropenem and ceftobiprole (8.9, 2.5, 24.4 and 13.9%, respectively). Whereas AMP disc testing can reliably predict susceptibility to amoxicillin, it showed very poor performances for other several β-lactams. Overall, aminopenicillins remain the first-line treatment, but MIC determination is recommended for accurate assessment of resistance to other β-lactams.
