A case report and literature review of multiple complications in the treatment of an aneurysm with the woven endobridge device

The Woven EndoBridge (WEB) device is proven safe and effective for wide-neck aneurysms in GCP trials; simultaneous intra-operative complications have not been reported.
Case introductionA 47-year-old man presented with sudden headache; CTA/DSA revealed a ruptured basilar aneurysm, SAH and moyamoya disease. WEB embolisation was planned. An incomplete aneurysm coverage required semi-deployment of a stent-assisted coil, followed by full stent deployment that pushed the WEB into the sac. Micro-thrombi on the WEB protrusion reduced posterior cerebral flow, which was reversed promptly with antiplatelet therapy. Post-op recovery was uneventful; no neurological deficits.
DiscussionWEB remains safe for wide-neck aneurysms, yet device sizing, release angle and irregular morphology heighten risk. Prolonged manipulation may provoke thrombosis and ischaemia; meticulous technique and rapid antiplatelet rescue are essential.
