<p>In the event of a&#xa0;mass casualty incident, the in-hospital care of critically injured patients with a&#xa0;low probability of survival is still inadequately regulated. Triage Category T4 (T4 = expectant) refers to patients who, due to poor prognosis and currently limited resources, do not initially receive acute medical treatment. The Department of Palliative Medicine at LMU University Hospital Munich was integrated into the hospital’s emergency response planning in order to develop, in collaboration with the responsible authorities, a&#xa0;care concept for this patient group. The goal is to provide the best possible palliative care to (T4) patients and psychosocial support for both their relatives and the healthcare staff involved. Specific structures have been established and incorporated into the emergency response plan. The focus is on the care and coordination of T4 patients, their families, and staff. The role of the Section Leader for Palliative Medicine &amp; Psychosocial Emergency Care ensures overarching and cross-site coordination. Additionally, defined alerting procedures, assignment of roles and responsibilities, medication and material stockpiles, as well as designated care areas, have been specified. Integration of palliative care into mass casualty response represents an important complement to acute medical care and psychosocial support, enabling structured operations and ensuring dignified end-of-life support even under extreme conditions.</p>
