<p>In situations where public emergency medical services are foreseeably delayed to respond to life-threatening emergencies, trained first responders with advanced emergency care skills can effectively and demonstrably bridge this gap in order to avoid serious deterioration. To be sufficient, organized extended first responder systems must be specifically qualified to meet the demands expected in their operational context. The medical oversight—such as that provided by, for example, company occupational physicians—should be determined by the employer in line with the German Mining Ordinance and clearly defined in contractual agreements. This ensures standardized quality control for equipment, essential medications, and ongoing training and education according to the applicable rescue algorithm. Such professionally equipped extended first responder systems can also be effectively deployed outside hazardous zones in remote areas with limited emergency service access.</p>
