Background <p>The necessity of having effective Hospital Alarm and Emergency Planning (HAEP) is particularly emphasized by increasing threats such as terrorist attacks or natural disasters. International experiences demonstrate that well-prepared hospitals are crucial for managing such events.</p> Objective <p>The aim of this study was to assess the current status of implementation and updates of HAEP, as well as the availability of structured training and exercise programs in German hospitals.</p> Methods <p>Data collection was conducted using a&#xa0;standardized questionnaire, which was sent via email to the medical directors of emergency departments in 1065 German hospitals across all levels of care.</p> Results <p>A&#xa0;total of 313 hospitals, represented by their emergency department directors, participated in the survey (response rate: 29%). Across all levels of care, 97.5% of hospitals currently have an HAEP in place. Over the past 2&#xa0;years, 70% of HAEPs have been updated, while 7.6% of hospitals reported plans older than 5&#xa0;years. Low-threshold internal training programs are available in 46.9% of hospitals, while specific training concepts are offered in an average of only 16.6%. Regular HAEP exercises are conducted in 66.3% of hospitals; however, full-scale exercises are particularly rare in nonuniversity facilities (on average, 24%).</p> Conclusion <p>The participating hospitals almost universally have an HAEP; however, significant differences were observed regarding the degree of updates. Substantial deficiencies were identified in the availability of low-threshold and specific training programs, the regular conduct of exercises, and the implementation of recommended personnel strategies. The consistent execution of comprehensive training and exercise measures, as well as ensuring adequate financial and human resources, are prerequisites for being well-prepared to provide appropriate patient care in the event of an incident, regardless of the level of care provided.</p>
