Innerklinische Notfallversorgung bei einem Massenanfall von Verletzten aus Sicht der zentralen operativen Notfallkoordination (ZONK)

As part of a response to a mass casualty incident (MCI), various concepts have been developed with the aim to optimize resources, treatment, and outcomes and are incorporated into various alarm and response plans in hospitals. In the prehospital setting, the MCI concept establishes the organization of space and time, coordinated by an experienced emergency physician with additional training for the leading emergency physician and an organizational emergency medical services (EMS) officer (organizational leader). In addition, the establishment of casualty collection points (CCPs) was implemented. A more recent extension of the concept mirrors these functions in the in-hospital setting with a chief physician for triage (“leitender Arzt der Sichtung”, LArS), a central operative emergency coordination (“zentrale operative Notfallkoordination”, ZONK), and the medical incident commander (“medizinische Einsatzleitung”, MEL). The example of the attack on the Christmas market in Magdeburg on 20 December 2024 is used to evaluate the implementation of this concept at the University Medical Center Magdeburg with a focus on interactions between LArS, ZONK, and MEL in the organization of casualty care.
